Audiometric Testing Center (PUA) – Mobile
Description/Responsibilities
The Occupational Safety and Health (Noise Exposure) Regulations 2019 state that every employer who has carried out a noise risk assessment and has shown that any employee is exposed to excessive noise in excess of the noise exposure limits specified in the regulations, the employer shall cause an audiometric test to be carried out on the employee annually. For the purpose of carrying out the audiometric test, the employer shall appoint an audiometric test centre approved by the Director General.
Responsibilities:
The PUA should:
- arrange an audiometric testing program with the employer;
- ensure that audiometric testing is carried out within the valid period of PUA registration;
- ensure that audiometric testing is only carried out on workers who have been in quiet conditions for at least 14 hours without PHP;
- notify the Director of the respective state office of DOSH at least two (2) weeks before conducting any audiometric testing program;
- submit the audiometric testing results report to the employer within 30 days from the date of the audiometric testing;
- inform the Director General of any changes to the approved PUA, including:
- appointed audiometric technician;
- Appointed OHD;
- PUA owner;
- static PUA location change;
- static and mobile PUA facilities;
- ensure regular maintenance and annual calibration of PUA facilities;
- maintain good audiometric testing records;
- and send a summary of all audiometric reports for each calendar year to the Director General of DOSH no later than 31 January of the following year

List of Mandatory Documents
- Audiometric test room design drawings certified by a vehicle inspection company appointed by the Malaysian Government
- Vehicle ownership certificate as a mobile service clinic from JPJ
- Valid calibration report for Type/Class 1 Precision Sound Level Meter (PSLM) and 'Sound Calibrator'
- Type/Class 1'by manufacturer or laboratory recognized by the Malaysian Standards Department
- Audiometric technician training certificate and appointment letter
- Certificate of registration and appointment letter of Occupational Health Doctor
- The audiometric test procedure includes the format of the test results
List of Supporting Documents
- Measurement procedures and calibration report formats for audiometric test rooms and audiometric measuring equipment
